Acne Scars, Texture & Enlarged Pores

How it happens

Inflammatory acne damages the pilosebaceous unit and surrounding dermis. As the lesion heals, abnormal collagen remodeling can create depressions (atrophic scars) or, less commonly, hypertrophic/keloid scars. Pores appear “larger” when follicular openings are surrounded by damaged or thinned dermal support. 

 

Variations

  • Ice-pick, boxcar, rolling atrophic scars.

  • Superficial textural roughness vs discrete deep pits.

 

Severity & treatment approach

  • Mild texture / shallow scars: microneedling, lighter peels, non-ablative laser, topical support.

  • Moderate scars: RF microneedling, deeper microneedling, TCA peels or focal TCA techniques, often in a series with PRP support.

  • Severe, mixed scar types: staged, combination approaches (RF microneedling + chemical reconstruction, sometimes referral for surgical subcision or resurfacing at higher levels of care).
